I/we, the undersigned adopting party, have fully read, fully understand
and agree to the following terms and conditions of this adoption contract
in the acquisition of the above mentioned dog.

I. I/we agree to adopt ___________ as of the date ___________. By adopting a
dog from HHRRT, I/we understand that I/we bear full responsibility for the
health & behavior of the dog for the life of the dog, which can be 15+ years. If
for any reason, I/we cannot or no longer wish to keep the dog, the dog can be
returned to HHRRT at any time. I/we pledge to never abandon the dog, or
surrender, sell, or give the dog to a pound, animal control facility, kill shelter,
pet dealer or store, laboratory, or dog auction. Initial: _______
II. I/we will provide adequate food, water, shelter, obedience training,
grooming, routine preventative veterinary care (including yearly exams and
vaccines, heartworm testing and prevention, flea & tick prevention, dental
check ups, etc.). I/we will provide any additional veterinary care as needed
should injury or illness occur; I/we will not allow the dog to suffer needlessly
and will provide comfort care at the end of the dog's life.
IIb. For Puppies under Six Months: Puppies will be altered between 6 - 8
months of age; altering will include any pre-surgical exam and lab work
deemed necessary by the veterinarian, as well as post-surgical pain
medication and incision care. I/we will send a health certificate testifying to
the surgery was performed and the puppy in good health. It will be signed by
the attending veterinarian, and mailed to HHRRT within 30 days of the
surgery at the following address: _________________________________

III. I/we understand that the dog has been microchipped for his/her own safety
and protection. After adoption, I/we will become the primary contact
individuals listed with the _______________ microchip company. I/we
understand and agree that HHRRT will remain listed as a secondary contact
for the life of the dog.
IV. I/we understand that rescue dogs have uncertain histories, and that any dog
rescued by HHRRT may have been neglected, abused, or mistreated and that
special care, precautions, or restrictions may be required, either temporarily
or for the life of the dog. HHRRT has given full disclosure about any aggressive
or fearful tendencies displayed by ___________ during his/her duration of
stay with an approved HHRRT foster home. However, HHRRT cannot
guarantee that any dog will be remain physically or behaviorally healthy
after adoption. I/we are taking the dog "as is" without any warranties.
V. I/we understand that all Northern Breed dogs, no matter how well trained,
have an instinct to roam if allowed to do so. Therefore, I/we will never allow
the dog to run off lead unless in a securely fenced enclosure, for the dog's own
safety. I/we will never allow the dog to run or walk off lead in public areas or
public parks.
Vb. If the dog has been reported or found running at large, either
intentionally or through unforeseen circumstances, HHRRT may
reclaim the dog upon proper notification. Initial: ______
Vc. If the dog has been reported or found running at large on multiple
occasions, HHRRT reserves the right to permanently reclaim the dog
and revoke all adoption rights, without financial compensation to the

VI. I/we understand that all Northern Breed dogs, no matter how well trained,
can become destructive, especially when lonely, bored, or frustrated. I/we
will not hold HHRRT legally liable or financially responsible for the
destruction of any personal possessions or property, or public property. By
signing this document, I/we agree to bear full responsibility - including legal
and financial responsibility - for the dog's behavior at all times.

VII. I/we understand that all Northern Breed dogs, no matter how well trained,
have high prey drives. No small animals (including but not limited to cats,
toy breed dogs, ferrets, birds, and rodents) will be entirely safe around the
dog if left unsupervised. I/we agree to not hold HHRRT legally liable or
financially responsible for the deaths or injuries caused by the dog to any
other animal after adoption. By signing this contract, I/we agree to supervise
any interactions between the dog and other animals, and to bear full
responsibility - including any legal and financial responsibility - for the dog's
behavior at all times. Initial: ______
VIII. I/we agree to notify HHRRT immediately if the dog has escaped, is stolen, or
becomes lost at the following phone numbers: ___________, _____________;
I/we agree to make all reasonable efforts to locate the dog.
IX. I/we agree to never allow this dog to be trained or otherwise used as an
attack dog, and to refrain from any and all activities that may result in
creating aggressive tendencies in the dog. I/we further pledge to never use the
dog as a "bait dog" to train aggressive tendencies in other animals.
X. I/we agree to use a properly fitted collar, with my/our name and contact
information affixed to that collar. I/we will affix any other tags (including
but not limited to county license, rabies tag, and microchip tag) to that collar.
I/we will check the fit of the collar regularly, to ensure that it is not too loose
that the dog can slip it off, or too tight to cause discomfort. I/we will never
leave a choke chain, or prong collar, on the dog when not attended.
XI. I/we agree to notify HHRRT at the phone numbers or email addresses
provided if any serious behavioral problems develop so that HHRRT can offer
assistance in handling these issues.
XII. I/we agree to notify HHRRT of the dog's progress. I/we understand that an
HHRRT representative will be contacting me/us to periodically check up on
the dog; particularly at 2 weeks post adoption, 4-6 weeks post adoption, and 6
months post adoption. My/our preferred means of contact is:

XIII. In the event of separation of households due to such things as divorce,
death, or other relocation, HHRRT must be contacted and informed as to who
will retain ownership of the dog. In the event that neither party wants or is
able to take the dog, the dog will be returned to HHRRT. HHRRT must be
contacted and allowed a reasonable amount of time (minimum of 72 hours) to
make accommodations for the dog in foster care. Initial: ___________

will be submitted to HHRRT as soon as reasonably possible.

I/we further understand that:
i. Noncompliance or failure to perform any of the above terms and conditions
may result in the immediate forfeiture of the dog and will warrant the
removal of the dog by HHRRT, and that a representative of HHRRT may
reclaim the dog at HHRRT's discretion, without payment of compensation to

ii. REPEATED NONCOMPLIANCE, OR NONCOMPLIANCE WITH MULTIPLE
TERMS LISTED ABOVE, IS CONSIDERED A MATERIAL VIOLATION, AND THE
DOG SHALL BE IMMEDIATELY RECLAIMED BY HHRRT. Initial: _________
iii. IF AT ANY TIME, I/WE OR ANY MEMBER OF MY/OUR HOUSEHOLD IS
CONVICTED FOR ANIMAL CRUELTY OR NEGLECT, THE DOG WILL BE
IMMEDIATELY REMANDED BACK TO HHRRT WITHOUT COMPENSATION.
I/WE UNDERSTAND THAT I/WE WILL BE IMMEDIATELY PLACED ON THE
NATIONAL "DO NOT ADOPT" LIST WHICH IS MADE AVAILABLE TO ALL
SHELTERS AND RESCUE ORGANIZATIONS IN THE UNITED STATES.

iv. I/we understand that the adoption fee of $________ is a donation, and is
nonrefundable, even if the adoption later proves to be unsuitable. This fee is
strictly to cover expenses paid by HHRRT for the veterinary health of the dog.

In consideration of the adoption of this dog, I/we agree to assume full
responsibility for the health, welfare, and conduct of this dog upon the date of
this contract. I/we further agree to hold harmless Husky Havoc Rescue, Referral
& Transport, any HHRRT representative or volunteer; and to indemnify such
organizations or individuals for any damages or costs resulting from liability,
known or unknown, anticipated or unforeseen, as a result of the dog's conduct
after the date of this adoption contract.
